The Personalized Health Program is a membership-based (concierge) healthcare program with an annual membership fee that covers enhanced services and access features not covered or reimbursed by Medicare or commercial insurance.

We believe these enhanced services support improved health, wellness, and an overall better healthcare experience.

We do not participate in commercial insurance plans.

For office visits outside of your annual comprehensive wellness appointment, you will receive a detailed invoice outlining services provided and fees paid. This can be submitted to your insurance company for possible out-of-network reimbursement.

After office hours and on weekends, Dr. Singh shares call coverage with Dr. Anne Rose Eapen, MD, FACP.

You will always be able to reach one of us, or during certain vacation periods, another trusted covering physician.

If you are hospitalized, the hospitalist service will serve as the attending physician of record.

Dr. Singh will remain involved as your personal physician, assisting with care coordination and being available for consultation with you and the hospital team.

After this conversation, a Personalized Health Program agreement can be provided for your review prior to your first appointment.

Choosing a physician is an important decision and our goal is to provide transparent, ethical, patient-centered care and ensure all your questions are answered before enrollment. We welcome your questions and encourage a conversation to determine whether our approach fits your healthcare needs.

Dr. Singh is a Medicare-participating provider. Medicare-covered services are billed directly to Medicare in accordance with Medicare rules and regulations.

Patients remain responsible only for applicable deductibles and coinsurance as determined by Medicare.
